ACKNOWLEDGEMENT OF PARTICIPATION & GENERAL INFORMATION
Fishing and related outdoor activities are enjoyable recreational experiences that take place in natural outdoor environments. Aaron’s Guide Service LLC operates in accordance with New York State safety requirements and places a strong emphasis on professionalism, care, and customer enjoyment. Participation in fishing, boating, and ice fishing involves natural conditions such as weather, water, ice, and terrain that are outside the guide’s direct control. This document is intended to ensure all participants understand the nature of these activities so everyone can enjoy a safe and positive experience.
RELEASE OF LIABILITY & ASSUMPTION OF RISK
I, the undersigned, in consideration of being permitted to participate in activities and services provided by Aaron’s Guide Service LLC, including but not limited to guided fishing, boating, and ice fishing, voluntarily agree as follows:
I understand that outdoor recreational activities involve inherent risks that may include, but are not limited to, slips or falls, exposure to weather conditions, cold or heat, sun exposure, dehydration, insects or wildlife, moving water or ice conditions, boating motion, use of fishing equipment, and transportation to and from fishing locations. While serious injury or illness is unlikely, I understand it is possible and I knowingly choose to participate.
I agree to assume responsibility for my participation and release and hold harmless Aaron’s Guide Service LLC, its owners, guides, employees, contractors, and agents from any and all claims arising out of my participation, including those resulting from ordinary negligence, to the fullest extent permitted by New York State law.
ICE FISHING & WINTER ACTIVITY ACKNOWLEDGEMENT
I understand that ice fishing trips take place on frozen lakes and involve winter conditions. Activities may include walking or standing on natural ice surfaces, the use of ice fishing equipment such as shelters, augers, tip-ups, heaters, and hand tools, and transportation of people or gear in sleds towed by a Snowdog or similar motorized utility sled across snow- and ice-covered surfaces.
I acknowledge that ice conditions, snow cover, and weather may change throughout the day. Aaron’s Guide Service LLC evaluates conditions regularly and will not operate in conditions it deems unsafe; however, I understand that ice is a natural surface and variations may exist. I agree to follow all guide instructions, remain within designated areas, and wear appropriate winter clothing and footwear.
PHYSICAL HEALTH & MEDICAL CARE
I certify that I am physically capable of participating in the activities offered and have no medical conditions that would prevent my safe participation. I understand that fishing and ice fishing may involve standing, walking on uneven or slippery surfaces, boarding or exiting a boat or sled, and extended time outdoors.
I understand that Aaron’s Guide Service LLC is not a medical provider. In the event of an emergency, reasonable efforts will be made to seek medical assistance. I agree that Aaron’s Guide Service LLC shall not be held responsible for the availability or quality of such medical care.
PERSONAL FLOTATION DEVICES & SAFETY EQUIPMENT
I acknowledge that U.S. Coast Guard–approved life jackets will be provided when required. I understand that I may be instructed to wear safety equipment while on the vessel, at launch areas, or during certain conditions. Choosing not to wear provided safety equipment against guide recommendations is done at my own discretion and risk.
MINORS
If I am signing on behalf of a minor, I certify that I am the parent or legal guardian of the minor and consent to their participation. I accept responsibility for the minor’s conduct and participation and agree to the terms of this waiver on their behalf.
PHOTO, VIDEO & MEDIA RELEASE
I grant Aaron’s Guide Service LLC permission to take photographs and/or video of me and/or any minor I am signing for during the trip. These images or recordings may include fishing activities and images of fish caught.
I authorize Aaron’s Guide Service LLC to use such photographs, video, or other media for promotional purposes, including social media, website content, advertising, and printed materials, without compensation. No names will be used unless separately agreed upon.
ACKNOWLEDGEMENT & SIGNATURE
I acknowledge that I have read and understand this document in its entirety and sign it voluntarily. I understand that this agreement affects my legal rights and is binding upon myself, my heirs, successors, and assigns.
